Supporting Evidence Docs

    I am just about ready to submit my fathers claim via the FDC Criteria and I want to be sure I am including all the necessary documents. My question is do I have to or should I send in the following documents:

    1. Bank Statements for all accounts
    2. IRA statement
    3. Pension and Social Security Statements
    4. What about the caregiver fee that my father paid to me via check, do I need to send a copy of a cashed check(s)?
    5. My father has a credit card bill, a small loan and a tax payment he makes monthly, on top of his other expenses, but I do not see anywhere to document this. Will these be taken into consideration for his expenses and should I provide back up docs to support expenses?
