Physicain statement blank left

    I am about to submit the 526 for my father + everything else.
    I just carefully re read the detailed Physician's Statement and saw that the Doctor neglected (probably forgot) to fill out Q#28 about Posture and General Appearance.
    May we write in what we see? Dad looks good for his age; somewhat stooped because he is always looking down to see where he is going. He needs a cane for balance and stability. He is dressed everyday and has to be meticulous about hygeine due to incontinence. He is coping with cardiovascular disease, diabetes, the balance issues, and needs 2 hearing aids.

    What else do we say about appearance etc.

    Sorry to say, but we recommend you get the Doctor to fill this in. I know it's tempting, but it is our recommendation.

    Don't fill it in yourself. That being said, the diagnosis and the questions on the first page are most important.
