Mom unable to read, comprehend or sign

    Completing paperwork for submission. My mother has very late stage Alzheimers and can no longer, talk, read or write. I am submitting a pkg including forms 21-592 (with several examples of severe dementia dignoses), 21-22a and 21-0792, and 21-2680 with current her current status. Question: I will be signing all forms where required including the 21-534, as she is unable to, is this the recommended method when the claimant is unable to sign?

    In the event that the applicant cannot sign applications and paperwork, they can mark with an X and have two people witness the form.
