Important Question Renewal

    My mother, after 14 months, countless phone calls, visits to the VA and extensive documentation and with the Congressman's help finally got her Aid and Attendance in June 07. She has been in and ALF since 9/04. The only other paper work included in the packet for renewal that was in her packet was Form 21-4706b, which is Federal Fiduciary's Account. I don't believe this is the correct form for a renewal. I saw on the website a similar questiona dn the response was to fill out a yearly EVE (Eligibility Verification Review). Is this the only form necessary that needs to be filled out? What is the form number?
    Can I download the form? If, so where? When does it have to be completed? Also, why would they send a Federal Fiduciary's Account form? I know the hardship all of this has caused and how long a wait period can entail. Looking forward to your response.

    Glad to hear the good news about your mother's approval for 'aid & attendance'. I will try to address your questions and please ask for any clarification that is needed. The VA will send out the EVR form on an annual basis. They are due on the anniversary of the approval date. If for some reason you don't get one, the VA will send your mother a reminder before any action is taken to terminate her benefits. If you want to take a look at the form so that you know what will have to be reported, you can look up form number 21-0581-1. This is for the Improved Pension Eligibility Verification Report (EVR). This form will have to be done each year. I am assuming your mother has no dependent children. There are several different EVR forms and it depends on a couple of things as far as which form is required.

    Were you appointed as fiduciary for your mother? If so, the form that the VA sent is a form to report what the beneficary's money was used for. I've not had any experience with this form. It appears as though this is an annual report form. I recommend you contact the fiduciary section with the VA Regional Office to get the details on this form.

    Claysee, I may have some good news for you but I want to check my reference material before I share the info with you. Be sure to check back in a day or two for additional info. I am sure it has been a long road to travel but it was worth the effort. Take care and thank you for not giving up.

    I was searching the forum for the renewal of A&A benefits process, and found this response which provides the information. However, I would like to confirm that the EVR form is due on the anniversary of the approval of the claim, and not the anniversary of the eligibility for benefits. Thank you.
