How to Document Cost of Terminated In-Home LPN?

    The last piece of this puzzle is how to document the cost of an LPN ($30k for working in-home) that my parents rightfully terminated in a prior year. Under the circumstances, there will be no affidavit from her.

    What alternative documentation might be accepted? Their computer ledger, cancelled checks, a statement from their doctor that he met and routinely consulted with this nurse on their behalf, and a copy of her license?

    Since you can't get her to sign, you would have to prove via checks, or the agency who she/he was contracted through.
