Form 21-0845

    I just received the newsletter and I'd like to thank you for the info. about the VA Form 21-0845. This will be a huge help in dealing with the VA, as my Dad has hearing problems (even with hearing aid). Would you know where we should send the form? We've been dealing with the Pension Benefits office in Philadelphia but I don't know if that matters...Thanks, again.

    Since you know which office is handling the claim, send it there, and send it Registered Return Receipt.

    Just so you know what to send, you would want send along would be the Statement in Support of Claim with the Dr's eval that they were impaired at that time and the medical expenses for the period.
