Doctor's form mistake. How to best correct?

    I didn't catch it that the doctor left box 10 blank. So the VA of course is asking for it. It is diagnosis. Instead of having to ask him to do it all over again, it was over 6 mos. ago, can I just send in the online notes from her last appt. that shows a diagnosis from mom's primary care doc? It's a different doc than who filled out the form. Or do I have to go back to the original doc who left box 10 empty?

    It depends on what the VA is asking for. If they are just asking for a Diagnosis (Dx) then you can just send them the notes showing the Dx. Otherwise I would just have the new Dr fill out the 21-2680 form again and send that in.
