This is my first post.  Hopefully I've done it correctly.  Can someone tell me how to fill in question 14 of form 21-22a "Appointment of Individual as Claimant's Representative" and what it means?  I'm not an agent or attorney.  I'm an individual (daughter) providing representation under section 14.630.  If I fill in question 14, does it meant that all previously existing powers of attorney are revoked (see parenthesis).  Thanks.

Thanks, again for the links.  But, the step-by-step guide to VA Improved Pensions does not address filling out application form 21-22a.  I don't know whether or how to fill out question 14 on the form.  Anybody know?

On the two different step-by-step guides, this veteran is my mom so the correct one for her is the one for veterans only.

There is a NEW VA Form 21-22a.  Use this one. If you are an individual representing the claimant, you need to have a statement certifying you are not getting paid to help the claimant.  If you send the old version, it will not be recognized.

The new 21-22a form can be found here:
