my father has a very limited history with his doctors.

he had brain surgery dec 2007 that resulted in a stroke, prior to that he did not go to the doctors.  he's now in an assisted living facility and refuses to go to any doctor.

the surgeon who preformed the surgery has agreed to provide a statement, but the forms i have (with the info the VA needs) asks when was the last time the physician saw the patient.  the last time the DR saw my father was may/june 2008.  i plan on submitting his application FEB or MArch 2009.  will this statement be good even if the  last time the DR saw my father was 6 to 8 months prior.

any info on this would be great

June/July of 08 will probably be fine.  A physical is not required, just a statement from an attending physician.  However, they can ask anyone they want to show up at the VA for a physical, so you might us that to convince your dad to go to the doctor - or have a doctor stop in to see him - most assisted livings have a relationship with one or more doctors who should be willing to do that.
