How long is the Doctor review form valid to submit with our Package

    Our 95 year old Mother was evaluated after an auto accident and the Doctor review formed filled in and signed. We have had a delay in moving on her entry into ASL or expanded homecare. How long is this evaluating Doctor's recommendations valid?

    Good question - it should be valid up to one year as long as her status does not change. But I am curious if her auto accident would have made her physician's statement different than her day-to-day life and needs since the auto accident could obviously impact her evaluation at that time.

    Thanks Kaylin .... Probably not because she had only a fractured right foot.
    Her physical conditions with Glaucoma and Lung issues are noted as well as Dementia
    His examination comments .....> Supervision and Monitoring of Meds and Vital Signs ....>Cannot prepare own meals.......>Cannot manage Financial affairs....
    ..>Posture/general appearance = Frail, Elderly, Unstable movement.....>Can leave home weekly for Medical visits, etc......> Does require nursing home care.

    Yep, then the fractured right foot would not be counted towards her ongoing costs of care. The rest of his evaluation sounds right and just for putting forth with the application for the A&A benefit.
