My Dad's address as listed on medicare/social security, etc is my home address. Moved into assisted living two weeks ago.  Will there be any problem if I use my address for his address on the Aid & Assistance application, if I have the form from the Assisted Living saying he lives there?

You can show his residence address, but all mail is received at your address.
