VA form 21-534 Section IX - repeat Medicare Deduction for each month?

First off,  thanks so much for this site.  I have been working with a consultant and was handed forms from 1998 to complete.  I had them mostly completed and came across this site and now I am filling them in via PDF with the 2009 forms.

I am filling out section IX of form VA form 21-534.  The instructions state to list the Medicare Deduction first.  Do I list it 12 times (once for each of the past 12 months)?

No, list it once and then in the next block write in "monthly" to indicate that this is paid each month.  Do this with each medical expense you have.

Just to add a note here, it has to arrive at the VA by the last business day of this month it to be counted for Aug.
