In the past week, I have been given completely different forms to apply for aid & attendance \3 different doctor forms. These were people that are suppose to know. I just read in your topics not to use the "Fully Developed Claim Notice, which was one of the three.  Can someone give me a list of all forms I will need.  If you just give me the numbers, I can look them up.
Applying for Aid & Attendance for elderly father(88). Meets  War qualifications with assets totally $39,000 and monthly income of $1900.  In nursing home at $3200 a month.
Mental ability is good,but very poor health and mobility.
   I have a power of attorney drawn up by lawyer, but should I also send a VA form for power of attorney?  I would appreciate a list of the most current forms required.  Thank you.

Faye,

You need to download the forms on the homepage of the site, as well as read the FAQ section regarding POA as the VA does not recognize it. You also need to include a 21-0845 and suggest you read the sucess stories on the forums.

For the physician's statement, use VA form 21-2680

For the application for a veteran, use form 21-526
