If a Veteran who has already been approved for care (12 hrs/week) moves to another state and therefore needs to use another care service provider, what needs to be done to transfer the payments from one provider to another?

You need to do a Statement of Support of Claim advising the VA of the applicant's new residence and the new provider.  Payments do not go directly to the company providing the care, so you would also need to notify of any banking changes for the deposit to be made.

Thank you so much for the information.  I thought the payments went to the provider.  Whose banking information must be supplied?  Can I get this form on the website?

Payments only go to either the veteran or surviving spouse, so whoever was approved for the homecare.  You can print off the Statement in Support here: http://www.vba.va.gov/pubs/forms/VBA-21-4138-ARE.pdf

I would also suggest to answer all of the questions I posted in the post titled "Care expense questions"

Does the Va assist a vet when he makes a move ffrom one state to another with his moving costs?

I am moving to another state to be with more family. Will the VA pay for my moving expenses?

Not with regards to this pension.  If there is some other VA benefit that covers moving expenses, we are not aware of it.
