I am in the processing of pulling together the documents I need to apply for A & A for my mother who receives care at home from a health care agency ' Seniors Helping Seniors" for her Alzheimers. On their letter head I have a statement including claimant's name and social secuirty #, citing the diagnosis, care given and why , the length of service to this point, the cost per month and the estimated cost over the next 12 months. Phone contact provided if further assistacne needed. We have had my mother pay privately by check directly to "Seniors Helping Seniors" based on the hours needed every 2 weeks. Do I need to send along all the cancelled checks for the past year or is this statement from the agency enough. Please advise.

For right now you can send the statement from the agency. At some point you may be asked for proof of payment, but showing the services provided and the costs for such is sufficient for now.
