Home Stretch - Direct Deposit

I've officially been appointed fiduciary, have opened an account and now must submit the Direct Deposit Form to have the monies deposited.  Section 3 of the Direct Deposit Form asks for Government Agency Name and Government Agency Address.  I assume it's the Department of Veteran Affairs but what address should I use?  The office where I submitted the application?
