On the VeteranAid website there's information about Power of Attorney ... I have found Form 21-4138 and filled that out.  The website includes the statement to make on Form 21-4138 ... HOWEVER it also mentions including Form 21-592.  I have searched the www.va.gov for such a form number and it doesn't seem to exist.

Am I missing something?  What is the correct Form to include with Form 21-4138?

Unless you are part of a recognized veteran's service org, you can't be POA for VA purposes.  You can be a fiduciary, but that is something VA sets up.  the 592 is a form VA uses internally for fiduciary assignments.  I'm thinking that what you got was a due process letter proposing to rate someone incompetent.  If the letter mentions something about the Brady Bill, then it is proposing to rate the claimant incompetent.  Submit medical evidence showing that the claimant is competent OR submit a waiver of due process by saying something like "I waive my right to due process and request a fiduciary to be assigned immediately."  you may use the 4138 for this
