I am applying for Aid & Assistance for my mother who lives in an assisted living facility in Ohio.  I live in Florida. Which state would be the best to send form 21-534 to?

You need to send for the state she resides in which would be:

Milwaukee Pension Maintenance Center
Veterans Administration
5400 West National Ave
Milwaukee, WI  63214
