MyHealth web site

I am a disable Veteran residing at the Calcutta Heath Care Center. My home VA clinic is in Calcutta, Ohio.
The VA has a web site called MyHealth. This site is set up to help veterans with their health concerns and prescriptions. To have full access to this site a veteran has to have someone at their home clinic validate their identity.

On December 3rd of 2009 I mailed the proper VA form to Ms. J. Simcox at the Calcutta VA clinic and ask her to process it so that I could have full access to the MyHealth site.
When I did not hear from her by Jan 6, 2010 I sent her an email asking how things were going.
She replied that while she was out of the office “one of her clerks had sent the form back to me because I had not included a zerox of my VA ID.”
Since this was a month after my application I ask why didn’t the clerk hold the form and Email me a request for the ID.
She did not reply.

I never received the form. I guess it got “lost in the mail”.

On Jan 18th I mailed another copy of the form and a zerox of my VA ID.
On Feb. 4th I emailed Ms. Simcox and ask her how things were going.
No reply.
On Feb. 10th I emailed Ms. Simcox and ask her how things were going.
No reply.
I have been looking at the site for years.
I have been trying to get full access since Dec. 3, 2009

I have been told by other Vets the site is quite helpful. I would like to use the site to monitor my prescriptions. Any help you could provide would be appreciated.
