I thought I was well along with this tedious paperwork until I received a letter from the VA requesting information regarding the provider of care:
 - the amount paid
 - The date payment was made
 - The nature of the service provided
 - the name of the person for whom services were provided
 - identification of the provider to whom payment was made
There is no problem in providing documentation for the above, but the home care aide only wants me to provide her name and no other information.  Will this suffice?

The VA is also looking for "supporting evidence" consisting of 1 of the following:
 - A receipt bill
 - Statement on providers letterhead
 - A computer summary
 - Ledger
 - Bank stmt (showing payments received)

Will I meet the VA rqmts if I have the aide sign a receipt for each weekly payment?  The aide does not work for an agency and all payments are made via check.

Appreciate your help.

1.  The full name of the care provider is sufficient.
2.  Copies of the checks are ok. That is easy enough to print off your bank's website, etc.
