Form 21-8416 won't print correctly

I downloaded the fillable form 21-8416 and filled it out,but it won't print correctly.The preprinted info comes out garbled
except the ones in italics.The data that I enter prints fine..

Other forms that I've downloaded print fine (10-10EZ,21-526).
I am not using VONAPP.

Any ideas?

technical difficulty? if it doesnt print with your home computer, try one in the library etc.  or call va and have them mail one to you.
