Well... here we go!

I am going to send in the application on tuesday.. (form 21-526_
I have a few final questions...

1.  My father (the veteran) is a widower. in section C, question 1, do i list him as "surviving spouse"?
    should i include my mothers death certificate?

2. In part D, section 6, i have more than 4 medical deductions to report.  Should i use an additonal sheet or use a separate medical expense report?

3.  In Part A, section 10, is there any need to include additonal remarks?

Eric,

If your dad is the veteran then, no he is not the surviving spouse.

Be sure to keep a complete copy of it and send it Registered Return Receipt.  What state does your dad live in so that we can give you the proper processing center to mail it to?

I sent out my Dad's application to Philly today via express mail.

Now the wait begins....
