I was told that I need to have a form completed by the Assisted Living to be submitted to the VA for Aid & Attendance benefits.   Mom moved out of the original assisted living community because she was not happy there.  She is currently in her second AA.  The original AA has refused to complete the forms for the VA.  We were told that you can only get benefits from the facility you are currently living at.   Is this true?    I thought it was from day one.   Also, would it suffice if we just sent copies of the original contract and copies of her monthly payments?

Please give me your thoughts.

yes, you might be able to use the contract and bills.
And the VA can go back.....

No it is not true that she can only get benefits if she were still in the original ALF.  She can live anywhere as long as she is still financially and physically eligible. 

The original faciltiy simply did not want to be bothered since you moved your mom out.  You can use the fees for the new place, let them fill out the paperwork and estimate her care by multiplying the monthly expenses.
