My father's application for Basic Pension was denied in June, 2009, but now he needs A & A. I need to know whether to just submit an EVR and Medical Expense Report for June through December (plus a Physician's statement, payment record from the assisted-living facility he is in) or whether to submit a completely new claim (Form 21-526, etc.). Two VA reps told me to just submit the EVR, etc., while one told me to submit a completely new claim.

Just do a Support of Claim showing the changes in his condition and monthly expenses.

i agree, send a statement as she said above.  an evr would be ok but in all liklihood you will end up getting a development letter in the mail with another evr to complete with specific date ranges on it. and a request for other things like verification of the veteran's ALF fees and medical evidence.
