ALF vs Nursing Home - Form

Just got a request for more information. I clearly stated Mom is in an Assisted Living Facility (ALF) yet VA returned Form 21-0779 Request For Nursing Home Information. A talk with the VA processing center in St Paul revealed that up until a month ago (Oct '09?) the VA had an ALF 'friendly' form. However, since it wasn't an "official" form, they've gone to using the Nursing Home form to obtain info from ALFs.
Anyone gone this route yet? Am looking for any words of wisdom to include to guide the ALF people.

the 0779 can be used to verify NH or ALF.
the point is that the VA needs to verify if the claimant is a resident/patient and medicaid (NH) and the level of care skilled or Assisted living level care.
