What is the next step?

My mother received a letter stating that the "evidence shows that the claimant needs a daily personal health care services of a skilled provder and based on the cited medical evidence, a rating of special monthly pension at the aid and attendance level is in order."
I am not sure what the next step is. She was sent a blank medical expense report form 21-8416. Are we to fill this out and send it in before we go about hiring a home health care provider? How will the funds be provided from aid and attendance to hire home care?
