I'm preparing to apply for the A&A benefit for my dad, an 88 year old WWII veteran, who's in an assisted living facility.  Do I use the "nursing home status statement" for an assisted living facility?   He has no specific disabling diagnosis, other than that his age and general health preclude him living alone.  Is he still eligible?

Also, the St. Louis records center indicates his records (and DD-214) were lost in a fire.  He has many other separation and service records.  Are there others that are acceptable substitutes?

Hi and Welcome...You'll find this forum a great help!
As far as your dad eligibility..you'll have to show that he "needs" the assistance of others for his daily needs, residing in an "assisted living" facility may qualify.....Read the above thread "Basic qualifications..."...and I'm sure others will give you more info on it.

My father's discharge papers (DD214) were also destroyed in the fire....Ask for the Form 13038...This is the discharge form they will want. Get several "originals"...you'll need them. They want an original sent with your application, and you'll definitely want to keep one. If you send a "copy" it has to be an "official" copy, "approved" by the VA....Duhhhh

Did you know that you can get a DD214 created by a county clerk?  I did this with my Dad's paper's since they could not find them in St. Louis.  It is free and they gave me 2 free certified copies.  Now they are on file in Tarrant County Texas and i don't have to try to contact St. Louis again.  Hopefully you have already gotten these from them, but if not, you can check into this.
