Dear Debbie,
Form  21-526, question 32 asks "Are you now living in a nursing home?".  My father-in-law lives at an Assisted Living Facility, memory care unit,  not a Nursing Home.  Do I answer "NO" and skip items 34B - 34-D ?
His ALF has filed out a "Care Expense Statement" giving the dates, cost, care, etc. and is signed by facility administrator that I intend to inclued with A&A Pension application.

    Thank you,
    Jag42 (Jeanne)

Just strike through it and write in Assisted Living.
