Advice needed on reimbursement for “Last Illness and burial”.  It seems every situation is slightly different…   
Here are the basic facts:
•  Father was WWII veteran, Mother died in October 2009
•  Filed for Aid and Attendance 10/28/10– lots of back and forth with VA.
•  My Father died 8/1/2011.
•  Received letter from VA on 8/3/11, dated 7/29/11 - Approved, retroactive to 10/28/10 - $1644/month pending fiduciary. (~$15k accrued)
•  Called the VA to report his death – asked about next steps to get accrued funds.
•  Response was that we would not get those accrued funds, and instead now we need to file for last illness and burial expenses.
•  Expenses (Medicare Part B, Part D, and Home Health Care) were paid through a Family Trust account.  We have copies of all of the expenses paid from 10/10 until his death.  (~ $35k)
•  To pay for expenses not covered by Long Term Care insurance while we were waiting for the VA A&A, we ended up cashing in several small life insurance policies and anything else we could cash in...
•  My sister loaned the Family Trust $10k to cover expenses while we were waiting.
•  My parents had a Life Insurance policy that pre-paid for the majority of his funeral services ($11k), and the Trust paid the balance of $1k.

At this point we’re trying to determine what we should file to give us the best chance of receiving reimbursement:
1.  All expenses from 10/10 until his death (paid out of the trust)? (~35k or at least up to the ~15k approved to that date by VA?)
2.  The $10k loaned to the trust by my sister?
3.  All of his burial expense ($12K)?
4.  The balance of his burial expense ($1K) paid by the trust?

Any help would be appreciated.
