We just received a response regarding Form #21-4138 that we did not oppose VA finding of incompetence for VA purposes for my father and that we asked that they proceed with the claim.  On that same Form we noted that I was requesting to be the fudiciary. 

Under the title - What We Decided states:  The evidence shows that your disability keeps you from ably managing your Department of Veterans Affairs (VA) benefits.  Therefore, we will find someone to manage them for you.

I have already requested on the original response to #21-4138 that I be the fudiciary.  Do I need to send another letter and what form should I send this request to be fudiciary.

Respond back noting that you are requesting to be appointed as his Fiduciary.
