The pension department is requiring me to complete form 21-0845 (Authorization to Disclose Personal Information to a Third Party) in order to get information on my Mother's claim. My Mother is unable to sign the form and we will sign with an "X". Thus the "X" must be witnessed. There are no spaces on this form for the witness signatures and information. Should we put this information in the margins of the form?

That, or they can sign next to the "X" since there should be space left on the line.
