I am going to apply for benefits for my dad who is 90 years old and suffering from dementia and other things. Can I sign the application for him? Most of the time he does not know who I am. Right now he is in a nursing home. I need to get moving on this application.

No, your dad has to sign. If he cannot sign his name, let him make an "X" and have it witnessed by two people. If he cannot do that, then get an ink pad and do a thumb print, and have that witnessed by two people.
