Forms 21-4138 and 21-0845

I am getting ready to file both of these forms and have a couple of questions. Filing for my mother who is a surviving spouse of a veteran. Is it correct that I put the veteran's name and social security # on the Form 21-4138 or do I use his name at the top and her social security # (using as my intent to file) and then on the Form 21-0845 I use her social security # on Question 5. I want to get this right and worried that I'm over-thinking it all.

4138 form you use the Veterans Name and SS.
0845 the Beneficiary or applicant. Not the Veteran.
