Glad to have found info on this site.  I'm completing forms for my dad and starting process as recommended with 21-4148 intent to file while gathering the rest of the necessary info and material.  He has dementia and at best may be able to make a mark somewhere in the vicinity of the signature block.  Is his "signature" required?  If yes, should I include witness signature(s) in the statement area of the form.  Or can my mother sign this form?

Hold your dad's hand and have him make and "X", and have it witnessed by two people.

Glad you found us, and make sure you mail to the proper Pension Center for his state.  Send Return Receipt, and keep a copy of everything for your records.

A thumbprint would also work in lieu of an X.
