Can someone let me know if the form 21-4138 is necessary to send with an application for Aid and Attendance, I have just about completed my packet to mail and want to know if anything should go along with the application. thanks Rickie

Richie,

If you are going to be overseeing the application process, calling to check on the staus etc, yes, you will need the 21-0845 "Approved 3rd Party".  Without it, the VA will not discuss the application with you.

I do have Form 21-0845 completed but is the Form 21-4138 needed also

Sorry......

Statement in Support of Claim is only if you need to add additional information.  If you have a completed application, it is not necessary.

If the VA comes back asking for and additonal information or documentation, you would use it to supply that information.

It can also be used to file an "informal claim" to start the clock.

It is good to use to explain your answer to question 35F.  The question on the application is a major headache for claims processors since it is under the section that is for monthly income, but the examples listed are for interest and "one time payments" which are inherently not monthly in nature.  It is good to use a 4138 or the remarks section of the application to explain your answer to this (i.e. if you write $100 in that question, write down the source and whether or not it is monthly or annual)
