Assisted Living Facility Statement for A&A application

Who out there knows where to find an ALF Statement to accompany an A&A application?

The VA does not appear to have a pdf form available on the internet.

Ask the ALF to complete a Statement of Occupancy.  They should have a form on hand.

Is this instead of the Nursing home status statement?

My folks are living at an ALF and I'm getting ready to apply for A&A so will they need form 21-2680 and the statement of occupancy you just mentioned?

The facility will have the Statement of Occupancy form.  They will list the monthly rate and services being provided. On the Nursing Home form you just cross out NH and put ALF.

So to be clear, I need the Nursing Home(changed to ALF) Status Statement & Statement of Occupancy and no Physician Statement?

Also, do I put the address of the ALF as their address? They still own their home and my sister lives there. I think this is the address that VA has.
