My father's family Dr filled out a short 2 page form (called Form 1 - Statement of Attending Physicain) and my father was also just seen for the 1st time by a VA dr who has turned in some sort of paperwork for his application for A & A.  My question is should I send this Form 1 (which has no number on it but states it is for determining need for A & A) along with the 526 application or let it stand with just the VA dr's assessment.

Go to the home page of the site and print off the 21-2680 http://www.veteranaid.org/docs/PhysicianStatementForA&A.pdf and get his dr to fill this one out.

It can't hurt to include it, as you never know where the one the VA dr did will end up, and this way you will know they have everything.

I would recommend using the VA21-527EZ application instead of the 526.  We are experiencing more timely approvals with this newer application.  If his spouse is living, also include the VA21-686c.
