We are applying for VA A & A pension benefits for my 90 + year old parents who are both WW2 Vets. I am curious and confused about the purpose of this form (21-4142) and if it's even necessary when submitting the 21-526 application. I understand that it grants authorization for the VA to obtain treatment records, but are they wanting a complete medical history along with all the doctors names, etc? The instructions mention a valid OMB control number. Maybe I'm missing something, but what is an OMB number? The OMB Web page noted on the form no longer exists.

We are about ready to submit our 21-526 application, all except for this form 21-4142. I would sure appreciate any help and guidance you might be able to provide.

It is the release so the VA can speak with their doctor - that is all - just the name and address of the doctor that signed the Physician's Eval.
