Please give me some help.  I have been working on the application forms for a few weeks, gathering all the necessary information and now have a few questions.
1. For form 21-2680 -does this matter which doctor fills this out.  Do we go to his primary care or the physical therapist?
2. My Mom is also alive -do we send in Form 234 with her information, she is the caregiver.
3. Where do we send the completed package, we live in Metairie, LA 70003
4. Form 21-4138 When and what is this form used for, do not fully understand.
Thanks in advance of any help you can give.

1)You want the primary doctor to fill it out as they are more familiar with the history.

2) If your mom is acting as the caregiver, that is indicative that she is not in need of assistance for her day-to-day living, so you are not filing as a veteran with a spouse.  Simply for the vet 21-526

3) Mailing address for the state of LA is:

Milwaukee Pension Maintenance Center
Veterans Administration
5400 West National Avenue
Milwaukee, WI 53214

4) Form 21-4138 is used for a number of things.  It can be used to file an "informal" claim to start the clock, and to submitt supporting information for an application in process or one that was denied and has new information that would change that status.
