VA form 21-8416 EVR annual eligiblity verification report

I have rec'vd this form for the first time for my mom. my questions are:
1. there are no dates filled in for the period i am to account for, should i assume
    it is intended for the year 2010?
2. are the expenses intended to net all her income to zero?- or just her SS    which is her only income other than the A&A.
3.  can i include the expense of new dentures, oral surgery, and eye exam, eyeglasses.. or are the expenses supposed to be ongoing monthly expenses?

I just want to make sure I fill this out correctly.

It should be for 2010.  To receive the max pension, you only need to zero out her non-A&A income, which would be SS in this case.  You can include all of those expenses you listed, but the VA will typically only count recurring expenses.  However, if she didn't have enough recurring medical expenses, it doesn't hurt to list the additional one-time expenses.  Hope this helps.
