Hi , my mother has alzheimers & I have read the different answers about which address to use.  I need it to come to me so I am going to use the "c/o" sugesstion here.  With the "c/o" do I put down my name as you typically would (ie: c/o jane doe 123 blank st.) or just c/o & my address only?  Also, on the 21-2680 doctors form would I put my address there also?
 
#25- should you answer "no" if a surviving spouse or leave blank?  I read somewhere where you should not leave blank answers?
#32a-if the answer is "no" do you leave 32c & 32d blank?

You can use c/o and your address but I would not do it if in a different state.
